Affidavit of Closure; BIR; Self-Employed to Compensation

AFFIDAVIT

I, _________________________________, Filipino, of legal age and capacity, residing at _________________, after having duly sworn in accordance with law, hereby depose and state that:

1.            1. I am a duly registered taxpayer with identification number __________________;

2.        2. I am currently gainfully and exclusively employed as ______________________ under the ______________________;

     3. As part of the requirements of the Human Resources Office, I was required to close my registration as ___________________with the Bureau of Internal Revenue;

       4. Upon inquiry with the Bureau of Internal Revenue, Revenue District Office No. ____, I was required to submit the following requirements, among others:

                  a.       List of ending inventory of goods, supplies, including capital goods;

b.      Inventory of unused sales invoices/official receipts (SI/OR);

c.       Unused sales invoices/official receipts and all other unutilized accounting forms (e.g., vouchers, debit/credit memos, delivery receipts, purchase orders, etc.) shall be physically submitted to the RDO where the Head Office is registered or where the Authority to Print (ATP) was secured.

5.       5. I am executing this affidavit to attest to the fact that from the time of my registration on ___________ up to the present, I never derived any income as a ___________________;

           6.  Due to the foregoing, I do not have any ending inventory of goods, supplies and/or capital goods. In addition, I have no unused sales invoices/official receipts as I never applied for an Authority to Print Sales Invoices/Official Receipts;

          7.  I am executing this affidavit to comply with the requirements for the closure of my registration as a _________________ as early as the taxable year __________ and for whatever legal purpose/s this affidavit may serve.
